The Fine Print:

• All payments are due at the time of service.
• Can not be used for purchase of retail dental products.
• Can not be used/combined with ANY insurance plan that we are already in network with.
• Courtesy fee reduction or included dental services are non-transferable.
• The Wellness Plan is based on a benefit year commitment, an no part of it is refundable
• Benefits valid only at one of The Foleck Center facilities
